Description:
A formal investigation, of which this report is a part, into the murder of Zahid Mubarek, in March 2000, in a prison cell in the in the Young Offenders Institution at Feltham, West London, where he was serving a 3 month sentance. Mubarek was murdered by another prisoner - a known racist.

In his forward, Ray Singh, CBE, Chair of the panel of Nominated Commissioners, states that the report found that 'failures of proper practice and management on the part of HM Prison Service' meant 'the environment at Feltham was neither healthy, decent or above all safe'.

The formal investigation into HM Prison Service of England and Wales took place under under the Race Relations(Amendment) Act 2000 with reference to the need to eliminate unlawful racial discrimination.
